Job Description
Residential Home Health and Hospice is seeking a passionate and dedicated Licensed Practical Nurse (LPN) to join their home health team serving Harrisburg, PA , specifically covering the lower half of Dauphin County . This is an opportunity to work with a respected healthcare provider offering a supportive environment and excellent benefits.
Location: Harrisburg, PA
Territory: Lower half of Dauphin County
Schedule: Monday–Friday, 8am–5pm
One weekend every 4–8 weeks (Saturday Sunday, 8am–5pm)
Pay Rate: $28–$29/hour
Bonus: $10,000 Full-Year Stay Bonus (FYS) for full-time status
Key Responsibilities:
Collaborate to provide management and delivery of patient care plans
Support patients in Harrisburg, PA and surrounding areas
Meet critical deadlines for charting and documentation
Self-manage time and schedule efficiently
Demonstrate experience using tablets, mobile phones, and EMR software
Schedule and Shift Details:
Work schedule and shift expectations will be provided during onboarding
Travel:
Occasional travel may be required within the local area
Required Qualifications:
Associate degree or higher
1+ year of experience in any of the following:
Electronic Medical Record (EMR) systems
Other EMR systems
TruChart EHR
Valid LPN license in Pennsylvania (PA)
Certification in at least one of the following:
American Heart Association Advanced Cardiac Life Support (ACLS)
American Heart Association Basic Life Support (BLS)
Valid driver’s license, reliable transportation, and auto insurance
Must not have applied to or worked with Residential Home Health and Hospice in the past 6 months
